2. The College requires that a full work history be submitted on the form provided: 

For Family Physicians 

For Specialists 

3. Complete the Credit Card Charge Form to charge $210.00 CAD to your credit card.  This is a new charge from the College for assessment of your credentials, refunded when you arrive in Saskatoon for your interview.  

4. Attach the completed Curriculum Vitae and Credit Card Charge Form to an email and send to cpssreg@cps.sk.ca or fax to +1 (306) 244-0090.
